Janet’s Planet, Inc. would like to use pictures/images or videos documenting various moments within the week of Astronaut Academy. In consideration of the above, I hereby give my permission and consent to Janet’s Planet, Inc. (1) photograph, film, screenshot, and video/audio tape my child, and, (2) to use and display images and sounds of my child on Janet’s Planet websites, archives, and promotional or informational material, including, but not limited to, newsletters, brochures, advertisements, and newspaper articles for its publicity and use to illustrate and promote the camp experience, and I hereby waive and release on behalf of my child and myself any rights to compensation for, or ownership of, such images and/or sounds of my child and the above uses of them by Janet’s Planet, Inc. I have read this audio/visual image release and agree to its terms and conditions.
